Event Request Form

After this form has been filled out, a staff member will review and place your event on the church calendar (now viewable on our website). We will get back with you regarding any changes regarding rooms requested.

For wedding requests, please contact our business administrator.

You will need to fill out a yellow custodial form (available in the Church office) for room setup, including equipment, tables and other things needed. Please do so at least 2 weeks before your event.
