Forms 

Please find the necessary forms for your child's enrollment for the 2012-2013 school year in the Lenexa United Methodist Church Preschool.

1) ALL children will need to fill out a Form A.

• Enrollment Form (Form A)

2) All children NEW to the school ALSO need to fill out Form B and Form C.

• Emergency Authorization Form (Form B)
• Kansas Department of Health Form (Form C)

3) Optional Forms

• Preschool Tuition Automatic Payment Form

The Emergency Authorization Form: Fill in the child's name on the top of the form and health insurance information at the bottom. This form must be notarized. If you don't have easy access to a notary, you can return it to me and I will notarize it for you.

Kansas Department of Health Form: Complete this form, including all immunization records. A Physician must sign the back of the form. The state of Kansas mandates that no child be allowed to start school without these forms on file.

Return the forms by August 1st. We appreciate your attention to this detail and look forward to a great start to the school year!
